Transcript Request Form
o Education Verification ($2.00 each)
Education Verification verifies that a certificate or diploma was received. The verification of education is sometimes requested by potential employers.
Transcripts are NOT issued for Education Verification requests
o Unofficial Transcript ($5.00 each)
Unofficial Transcript does not include official seal and signature. An Unofficial Transcript is sufficient for personal records
o Official Transcript ($30.00 each)
Transcript request will not be processed until payment is received. Official Transcript includes official seal and signature. An Official Transcript is usually required to transfer credits to another school or for employment purposes.
o Official Diploma ($30.00 each)
Official Diploma will signify the graduation status and will be signed by the current campus management team at the time of issuance. ONLY for students who previously received a diploma at the time of graduation.
